You have the right to obtain copies of your medical record. We take great care to keep your medical records confidential and secure. Except where permitted by law, your medical record cannot be released without your written consent.

In-Person Pick Up

Once notified that the records are ready for pick up, patients and representatives can visit 2424 W Chicago Ave, Unit 102, Chicago, IL 60622 - By Appointment Only: to schedule an in-person pick up appointment, pease call (773) 276-6831. Photo ID is required. Records will be provided printed or on a memory stick. Please note, visit to the clinic is not a necessary step, records can be received online via secure link.

    • The patient or the patient’s legally authorized representative (legal documentation is required)

    • The parent of a patient under the age of 18

  • You do not have to pay for records that we send directly to you or to another medical professional.

    If you ask us to send your records to someone who is not a medical professional, such as an attorney, we may bill that person.
